Cancellation Policy - There will be a $15 fee for any appointment canceled with less than 24hr notice
In-Home Visits $150 - Includes well, sick, and injury visits. See locations above for areas we are servicing.
In-Home Visit for Additional Child (Same Household) $100 - Let us see all of the kids at once. $100 for each additional child
Virtual Visit $75 - A video call with one of our providers. Not all conditions can be diagnosed. Example of diagnosable conditions: pink eye, some rashes. Example of undiagnosable conditions: respiratory illness, sinus infection, ear infection, Strep. If you have any questions please call/text us 480-877-9899
Phone Call $25 -
Provider will call you on the phone to discuss your concerns for up to 10 mins. This is for questions only. Ie: feeding questions, questions about over-the-counter medications, if an ER Visit is needed, etc.
*NOTE* The provider will NOT be able to diagnose anything or prescribe any medications.
Sports Physicals $100 -
Please have all paperwork ready when the provider arrives
Covid-19 Rapid Antigen Test Administration - $100
We have nasal swab COVID-19 rapid tests. The test takes approximately 15 mins to read. Once results are read by the provider a signed letter with the results will be given to the parent. All test results will be reported to the state and county within 24hrs.
*This does not include a physical exam. If you would like your child examined by the provider please add an In-Home visit also.
Additional Covid-19 Test Administration (Same Household) - $75
When scheduling Multiple Covid tests: For one test select "Covid Test Administration". If you need 2 or more tests add "Additional Covid Test Administration" and either put in the notes the total number of tests needed or text it to us at 480-877-9899.
