In-Home Visits available for:

Tempe, Mesa, Chandler, Gilbert, and Queen Creek.

Virtual Visits and Phone Calls available for all of Arizona​​​

Cancellation Policy - There will be a $25 fee for any appointment canceled with less than 24hr notice

​

 

In-Home Visits $150 - Includes well, sick, and injury visits. See locations above for areas we are servicing.

In-Home Visit for Additional Child (Same Household) $100 - Let us see all of the kids at once. $100 for each additional child

​

Virtual Visit $75 - A video call with one of our providers. Not all conditions can be diagnosed. Example of diagnosable conditions: pink eye, some rashes.

Example of undiagnosable conditions: respiratory illness, sinus infection, ear infection, Strep.

Phone Call $25 - 

Provider will call you on the phone to discuss your concerns for up to 10 mins. This is for questions only. Ie: feeding questions, questions about over-the-counter medications, if an ER Visit is needed, etc.

*NOTE* The provider will NOT diagnose or prescribe any medications.

Sports Physicals $125 - 

Please have all paperwork ready when the provider arrives

Strep Test - Included in cost of visit

In-Home Adhesive Wound Closure/Stitches $275 (includes visit)

Abscess Drainage $75

 Tongue Tie removal (under 1 month old) - $75

Ingrown Toenail Removal $250 (includes visit)
